Directions: Please go to the 4th Floor of the Calder Plaza Building, 250 Monroe Ave. NW, downtown Grand Rapids, 49503. This is the reddish brick building with a lot of glass right across from the Devos Performance Hall entrance on Monroe Ave (just south of Michigan Street and just North of Pearl Street). You’ll see Bank of America and Roam restaurant at the front entrance of our building.
Parking: Best parking is the city/county-parking garage (look for green sign off of Monroe Ave. right across from the Devos Convention Center) @ $2.65 an hour.
Payment: A payment of $130.00 to $150.00 is required before or at the start of the first session using cash or check even if you plan on utilizing insurance for your first session. If not, we'll have to reschedule our session. You may also opt to pay with a credit card on this website. You can make a payment using PayPal ahead of time. Go to the payments tab and click on it. Bruce will refund your payment once your insurance remits payment for the first session or apply this money to any future copays.
Special Note: The lobby doors on the 4th floor may be locked after 5:00 p.m. If someone let’s you in, please have a seat in the lobby. If not, please remain in the hallway and have a seat there. Bruce will be with you as soon as he's finished with the session prior to your session. If you’d like his office to bill your insurance, please bring two copies of both sides of your insurance card and drivers license.
Forms: Click on the Useful Forms and Privacy/Confidentiality tabs on this page to complete and download the necessary paperwork. Print the forms and bring to your first session. If you are not able to do this, it's not a problem. You'll receive these forms at the end of the session to fill out.
